Fistula Salvage

Non-maturing arteriovenous fistulas (AVF) will be lost without early intervention, ultimately leading patients to have prolonged use of a central venous catheter for their hemodialysis. Up to 60% of AVFs fail to mature.

Fistula Salvage

Case History

Condition: 4 month old brachial artery to cephalic vein fistula

Patient: 34 year old male

Chief Complaints: Poorly matured fistula, unable to cannulate, and attempted cannulations have resulted in hemotomas.

Referring Physician: Nephrologist

Requested Services: Assess reason for cannulation difficulty and assist with maturation, if possible, and catheter evaluation.


Treatment

We have developed a fistula maturation technique comprised of a series of angioplasties of small and medium sized vessels in order to develop a fistula which is easy to cannulate for hemodialysis.

Initial Visit: Cephalic vein stenosis: entire length of vessel angioplastied using 8mm balloon.

72 Hours: Angioplasty of arterial anastomotic stenosis using 3mm balloon.

1 Week: Final angioplasty of arterial anastomotic stenosis using 5mm balloon.

29 Days: Fully matured fistula.
